Youthful Aging Home Care’s Geriatric Care Management helps older adults with chronic conditions and complex health needs. Our in-home care nurses work over the longer-term with clients, and their families if appropriate, to facilitate the continuity of care that is critical to well-being.

Most important, the home care process we follow promotes clients’ self-management and has been shown to improve patient care while increasing physicians’ satisfaction with chronic care.

The in-home health care nurse:

• provides an in-home care assessment and care plan
• monitors patients’ medical conditions
• facilitates good communication and coordinated care across all of the senior’s health care professionals
• provides information and support to their family as needed
