Our RN Care Management program gives clients and families a dedicated nurse who oversees health needs, coordinates care and offers expert guidance through both routine and unexpected changes. It provides clinical structure, confidence, and support when it matters most.

Our RN Care Managers understand each client’s complete health story and serve as a knowledgeable advocate. We bridge communication between families, caregivers, and providers so care stays coordinated, thoughtful, and centered on what each client needs to remain safe and supported at home. Clients with high-risk chronic conditions and complex health issues can receive closer medical oversight through our Johns Hopkins Guided Care program. Additionally, our RN’s provide Care Management Services to help seniors proactively take control of their health with intervention before symptoms progress.
RN Care Management includes a one-time enrollment fee, followed by an hourly rate for ongoing services. Clients may choose weekly or monthly billing based on their preference for transparency and financial flexibility.
